BLUE CROSS BLUE SHIELD - CARE FIRST
I am a provider for Blue Cross Blue Shield Care First, PPO and HMO. Please call your plan prior to your first appointment to inquire about mental health coverage and your co-pay. After our meetings, I will submit the claims. You may have a co-pay which you will pay at the end of the session.
ALL OTHER INSURANCE CARRIERS
If you are a member of a different insurance plan, please inquire if you have out of network benefits. As a licensed counselor, my invoices can be submitted to your insurance company for partial out of network reimbursement. Please note that you will be responsible for paying me and submitting your invoice to your respective insurance carrier.

Initial consultation: $150
Individual psychotherapy session: $200
Parent appointment: $ 150.00
